Staffed by the professionals of Premier Rehab, The Oaks Rehab is able to create therapy programs tailored to each resident to help ensure that appropriate functional skills are identified, learned, safely implemented and reinforced.

Which residents qualify for therapy?

The Oaks Rehab BACK 2 HOME Program is designed to help certain patients meet their ultimate goal of getting back to a normal life. Our specialized home re-entry program serves individuals requiring additional care while recovering from injury or illness.

We provide more extensive treatment options than acute hospitals or other inpatient rehab facilities can offer. Many patients simply are not ready to return home safely at the time of discharge from a hospital setting. With that in mind, this program is designed to offer more intensive physical, occupational, and/or speech therapy while incorporating therapeutic interventions to prepare you for integration back into your daily routine… at home.

Which residents qualify for therapy?

1. Patients who have Medicare Part A benefits.

2. Patients who have Managed Care Part A benefits.

3. Patients who have had a three-night hospital stay within the last 30 days.

4. Patients who can participate in at least 30 minutes or up to three hours of therapy per day.

Diathermy is a therapeutic treatment for muscle and joint conditions that uses a high-frequency electric

current to stimulate heat generation within body tissues.  The heat stimulates increased blood flow to an area, relieves pain and improves the mobility of tissues as they heal.

Omnicycle is a motorized rehabilitation system that facilitates upper and lower extremity exercise for all

patients, including those who are unable to use traditional exercise equipment, allowing for earlier

therapeutic exercise intervention.

Omni VR

Omni VR

Omni VR is a 3D virtual rehabilitation system designed for aging adults and others with physical limitations.  This technology uses leading-edge rehabilitation technology, including a 3D camera and specialized computer software, and allows the therapists to customize exercise programs specific to each patient’s needs.

PENS

PENS (Patterned Electrical Neuromuscular Stimulation) is a therapeutic treatment for urinary incontinence, pain management and neuromuscular reeducation and treatment of muscle disuse atrophy.
